Isolator
SG-O·Isolator
About the Isolator PCB
Isolator is an open source PCB design by SG-O, published on GitHub under the MIT license. It is a 2-layer board measuring 33 × 67.3 mm, with 35 components from 12 distinct parts.
Other key parts include the ISO6741 and ISO6721. By type, the board carries 14 resistors, 12 capacitors, 6 connectors and 3 ICs.

Tripple Isolator for two Step/Dir/En and one UART signal
This was designed to be used with the ODrive motor controller. It may be useful for other things though.
You can find the manufacturing documentation here.
